Adoption Application & Agreement Forms

Prior to meeting any of the dogs in our rescue program, we ask that you submit an adoption application. This will help us learn more about you, your situation, and what kind of dog you would like to make part of your family, thereby assisting us with making the best match possible between dog and adoptive family. If you are interested in a specific dog, please note its name on your application.

Please note that as a policy ETBCR does not place puppies in families with children under 6 years of age, as border collies and border collie mixes can be very reactive to the movements of small children.

(Our current adoption fees range from $125 to $150. Rescue dogs are spayed/neutered, vaccinated, checked for heartworms and intestinal parasites and treated if necessary, on heartworm preventative, treated with flea/tick preventive, and microchipped).		 YesNo
